Disability Support Coordinator sought to join the team at Progressive Care UK Limited in Sheffield. As a key member of our multidisciplinary team, you will play a vital role in delivering exceptional care and support to young people with learning disabilities.

About the Role

As a Disability Support Coordinator, you will work collaboratively with colleagues to develop and implement individualized care plans that meet the unique needs of each resident. Your expertise and experience will enable you to ensure the safety and well-being of residents, while also promoting their independence and self-advocacy.

Salary: Up to £29,720 - £37,852.80 per annum (£13.25 - £17.16 per hour) based on an average of 42 hours per week.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with the multidisciplinary team.
  • Conduct regular risk assessments and reviews to ensure resident safety and well-being.
  • Provide guidance and mentorship to staff members to promote excellent care delivery.
  • Maintain effective communication with the management team to ensure high standards of service delivery.

Requirements:

  • A Level 2 Adult Care or Health and Social Care equivalent qualification.
  • A Level 2 Team Leader or management qualification, or the willingness to obtain one post-appointment.
  • Proven experience in providing care and support to individuals with learning disabilities.
  • Strong communication and interpersonal skills.
