We are seeking a skilled Community Registered Mental Health Nurse to join our Great Yarmouth and Waveney Access and Assessment Team (AAT). As a key member of our team, you will play a vital role in providing high-quality care to individuals with mental health needs.
Key Responsibilities
- Undertake assessments, planning, and delivery of direct patient care with minimal supervision.
- Communicate decisions about mental health care services, referrals, and future support arrangements to individuals with mental health needs.
- Accurately record all patient support, including assessments, reviews, day-to-day contact, and discharges.
