Job Overview

This role involves working with unpaid carers to identify their needs and provide tailored support. The successful candidate will have experience of working in health or social care and be able to communicate effectively with carers and professionals.

Core Responsibilities:

  • Supporting carers through the Carers Assessment process and developing Carers Support Plans.
  • Providing carers with information about available services and facilitating referrals.
  • Liaising with colleagues in statutory services to ensure effective service delivery.

Skills and Qualifications:

  • Relevant qualifications in health or social care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic IT skills, including Microsoft Office and Outlook.

Salary Details:

£26,753 per annum, paid weekly.
