About the Job:

You will work closely with our multidisciplinary team to assess and meet the complex needs of our residents, developing and implementing individualized care plans that promote independence, dignity, and quality of life.
Main Responsibilities:
Deliver high-quality care and support to residents, addressing their physical, psychological, and social needs through comprehensive care planning.Collaborate with the care team to identify and respond to emerging issues, prioritizing resident safety and well-being.Develop and implement individualized care plans that reflect the unique needs and goals of each resident, ensuring timely and effective interventions.Monitor and evaluate the effectiveness of care plans, adjusting strategies as needed to optimize resident outcomes.Promote a culture of excellence and continuous improvement within the care team, sharing best practices and innovative solutions.What We Offer:
We offer a competitive salary and benefits package, including a £2,000 welcome bonus, ongoing training and development opportunities, and a supportive and collaborative work environment. Based on the estimated salary range for this role, we anticipate offering a salary between £38,000 and £42,000 per annum, depending on experience and qualifications.
