Job Summary:
We are seeking a highly skilled and experienced Care Coordinator to join our team at TRCGroup. As a Care Coordinator, you will play a vital role in supporting our clients in their homes, providing high-quality care and services that meet their individual needs.
Key Responsibilities:
- Implement and maintain an effective quality assurance programme to promote high-quality care and services.
- Develop and maintain strong relationships with clients, families, and healthcare professionals.
- Conduct thorough assessments of clients' needs and develop personalized care plans.
- Lead and manage a team of care professionals to ensure high-quality care delivery.
- Monitor and evaluate the quality of care services, identifying areas for improvement and implementing changes as needed.
- Collaborate with other departments to ensure seamless service delivery and high-quality outcomes.
Requirements:
- Minimum of 2 years' experience in a health and social care setting.
- At least 2 years' relevant experience in an operational management capacity in a health and social care setting.
- Experience of working in/managing a domiciliary service.
- A knowledge and understanding of the current legal responsibilities and standards of the service, including the need for the management and delivery of person-centred services.
- Full driving licence with use of own vehicle.
- Ability to provide advice and guidance outside of normal office hours as required.
- Leadership qualities.
- IT literate including basic Excel.
- Ability to effectively manage the performance of others.
Desirable Qualifications:
- NVQ Level 4 in Leadership & Management or the equivalent (or willingness to work towards the new diploma) - Level 3 Minimum
