We are seeking a highly skilled and motivated Band 5 Community Nurse to join our team at Hounslow and Richmond Community NHS Healthcare Trust.
About the RoleThis is an exciting opportunity for a dedicated and compassionate nurse to work in a dynamic and supportive environment, providing high-quality care to patients in their own homes.
As a Band 5 Community Nurse, you will be responsible for delivering holistic care to patients with complex needs, working closely with specialist nurses, allied health professionals, and social care colleagues to ensure seamless care.
You will be expected to work independently, assessing patients' needs, developing and implementing individual care plans, and evaluating their effectiveness.
Key Responsibilities- Provide high-quality, skilled nursing care to patients in their own homes
- Assess patients' needs and develop individual care plans
- Work closely with specialist nurses, allied health professionals, and social care colleagues
- Deliver care that is patient-centered, evidence-based, and meets the needs of the patient and their family
- Collaborate with the Community Nursing Team Manager to ensure effective care delivery
- Level 1 Registered Nurse with valid registration with the NMC
- Experience of nursing at Level 5 or evidence of a back to nursing programme
- Experience of working in the community setting
- One year's post-registration experience in a relevant setting
