About the Role

We are seeking a skilled Health Promotion Specialist to join our Falls Prevention Team within Livewell Southwest Wellbeing services. As a key member of our team, you will contribute to reducing falls and fall-related injuries across Plymouth, supporting older adults to live their best lives.

Key responsibilities include conducting multifactorial falls risk assessments, carrying out one-to-one interventions for behavior change, and liaising with partners to help reach target populations and communities. You will also actively seek out Hard to Reach populations and help them engage in healthy lifestyle initiatives.

As a Health Promotion Specialist, you will contribute to the delivery of falls prevention training programmes for other professionals and contribute at network meetings and team events. You will also give feedback on performance and local intelligence to senior management team.

We are looking for a candidate with knowledge and understanding of the psychology of behaviour change, knowledge of the benefits of health improvement interventions, and understanding of determinants of health and health inequalities.

Additional requirements include a full current driving licence and access to a car or other appropriate form of transport, as well as flexible hours.
