Job Summary

We are seeking a skilled Family Health Worker to join our team at Livewell Southwest. The successful candidate will work with families, primarily those with children aged up to 5 years, in a variety of settings.

About the Role

The postholder will be responsible for delivering episodes of care, as delegated and assessed by the SCPHN, in response to identified need. This will involve working in partnership with families, health professionals, and other agencies to promote healthy child development and prevent adverse effects on well-being.

The postholder will also be required to identify deviations in normal child development and refer to the SCPHN responsible for delegation and plan of care. In addition, they will deliver and evaluate individual, clinic, and group activities in response to identified Public Health need.

Main Responsibilities
  • Deliver planned episodes of care to families in their own homes, clinics, and nurseries
  • Support HVs and complete universal assessments in clinics and nurseries
  • Deliver public health messages and offer health education groups across the locality

  • Be available to work outside normal working hours, occasional evenings and Saturdays for specific health promotion activities
Salary and Benefits

We offer a competitive salary of £25,000 per annum, plus benefits including protected CPD time, various development pathways, leadership and mentoring programmes, and access to funding for training.
