Job Summary:

The Clinical Nurse Specialist will provide high-quality nursing care to adults in a nursing home setting, ensuring compliance with regulatory standards and best practices.

This role requires a professional who can maintain dignity, comfort, and respect for residents while overseeing all aspects of care planning and delivery.

Key Responsibilities:
  • Clinical Care:
    • Provide high-quality nursing care tailored to individual resident needs, including care planning, assessments, and reviews.
    • Monitor, assess, and evaluate resident health and well-being, escalating issues to medical professionals as necessary.
    • Ensure care provided aligns with current best practices and complies with national standards and guidelines.
    • Promote a person-centred approach to care that enhances resident well-being, respecting their choices and preferences.
  • Medication Administration:
    • Administer prescribed medications accurately and in line with NMC standards and the nursing home's policies.
    • Ensure medication administration records (MAR) are maintained accurately and promptly.
    • Monitor residents for adverse effects or reactions to medications, reporting concerns to the GP or relevant healthcare professionals.
    • Carry out controlled drug audits and ensure medication storage and handling practices meet regulatory standards.
  • Care Planning:
    • Develop, implement, and review care plans in collaboration with residents, their families, and the multidisciplinary team.
    • Conduct regular reviews and assessments of residents' care plans, ensuring they are updated in response to changing needs.
    • Involve residents and their families in decisions about care plans to ensure care provided aligns with their wishes and needs.
  • Leadership and Supervision:
    • Provide leadership and support to care staff, ensuring care is delivered effectively and professionally.
    • Ensure all care staff adhere to best practices, policies, and regulations.
    • Assist with the orientation and mentorship of newly appointed care staff and provide ongoing training as required.
    • Act as the shift lead when required, ensuring the smooth running of the unit and resident care is maintained at all times.
  • Compliance and Documentation:
    • Maintain accurate and comprehensive records in accordance with NMC guidelines and the nursing home's policies.
    • Participate in audits, inspections, and assessments to ensure compliance with CQC standards and other regulatory requirements.
    • Ensure safeguarding and health & safety policies are strictly followed, identifying and reporting any risks or incidents as necessary.
    • Uphold confidentiality and data protection protocols, ensuring residents' personal information is managed securely.
  • Communication and Teamwork:
    • Communicate effectively with residents, families, and healthcare professionals, fostering collaborative relationships to enhance resident care.
    • Liaise with GPs, social workers, and other healthcare professionals to ensure a multidisciplinary approach to care.
    • Participate in team meetings, reviews, and care discussions, offering input on clinical decisions and resident well-being.
    • Provide emotional support to residents and their families, addressing any concerns or questions with sensitivity.
  • Professional Development:
    • Maintain active NMC registration and comply with all revalidation requirements.
    • Engage in continuous professional development to keep up-to-date with nursing practices and new healthcare developments.
    • Attend in-house training sessions and workshops to enhance skills and knowledge, contributing to personal and professional growth.
Qualifications and Skills:
  • Essential:
    • Registered Nurse (Adult) with a valid NMC Pin.
    • Proven experience in a clinical or nursing home setting.
    • Strong knowledge of CQC regulations and the NMC Code of Practice.
    • Ability to manage medication administration in a safe and timely manner.
    • Excellent communication and interpersonal skills.
    • Demonstrated ability to lead, supervise, and work effectively within a multidisciplinary team.
    • Compassionate, patient-focused, and committed to upholding residents' dignity and rights.
  • Desirable:
    • Experience in elderly or dementia care.
    • Additional clinical qualifications or specialist training.
    • Experience in care plan management and auditing.
