About the Role

This exciting opportunity is for a Trainee Advanced Clinical Practitioner to join our team in Urgent Community Response and the Frailty Hospital at Home across Mid Hampshire. As a trainee, you will work under supervision to develop a high degree of autonomy and complex decision-making skills, underpinned by a MSc level award. You will manage clinical care in partnership with individuals, families, and carers, analyzing and synthesizing complex problems to enhance people's experience and improve outcomes.

Main Responsibilities
  1. Assess, diagnose, and treat acutely unwell people at risk of hospital admission.
  2. Receive supervision and development from qualified advanced clinical practitioners and consultant practitioners.

Person Specification
  • Appropriate professional qualification allowing registration with the NMC or HCPC.
  • Professional UK registration with the NMC or HCPC.
  • Good command of the English language, both written and verbal.
  • Computer literacy in data entry and IT communication.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
Experience
  • Evidence of developing specialized clinical skills in frailty, urgent response, and hospital at home care.
  • Evidence of implementing change to promote high-quality practice.
  • System working with individuals, families, carers, communities, and others for population health and reducing inequalities.
  • Contribution to service development, e.g., applying quality improvement methodology.
  • Awareness of local and national agendas across the area of specialty.
