We are seeking an experienced and motivated Senior Reablement Worker to join our team within the city-wide Reablement Service. As a Senior Reablement Worker, you will play a key role in supporting individuals to regain their independence and confidence after an illness or fall.
Key Responsibilities- Supervise and manage a team of Reablement Support Assistants
- Assess and develop care plans to promote independence and confidence
- Work closely with service users, families, and healthcare professionals to achieve desired outcomes
- Contribute to the development and implementation of service policies and procedures
- Experience in providing care to vulnerable adults in the community
- NVQ/QCF Level 3 or equivalent qualification (or willingness to undertake training)
- Effective communication and interpersonal skills
- Ability to work independently and as part of a team
