Job Description

Permanent, Full Time 37 hours per week.

We are seeking an experienced care or reablement worker to join our established team within the city-wide Reablement Service. As a Senior Reablement Worker, you will be part of the process that supports people to leave hospital earlier, after an illness or fall which has affected their confidence and ability to carry out the activities of daily living.

You will work with people for a period of up to 42 days to promote their confidence and maximise their independence. Your role will involve supervising reablement support assistants and managing the care element of the service users' time with the service.

Key Responsibilities:

  • Provide high-quality care and support to service users in their own homes.
  • Supervise and motivate reablement support assistants to ensure they are providing the best possible care.
  • Manage the care element of the service users' time with the service, ensuring that their needs are met and their independence is maximised.
  • Work effectively as part of a team to achieve the service's goals and objectives.
  • Communicate effectively with service users, their families, and other professionals to ensure that their needs are met.

Requirements:

  • Experience in providing care to vulnerable adults in the community.
  • NVQ/QCF level 3 or equivalent qualification or willingness to undertake training to complete this whilst in the role.
  • Effective communication skills and ability to work effectively as part of a team.
  • Ability to supervise and motivate staff and complete assessments and set/review goals with service users in their own homes.
  • Flexible response to changing needs and ability to work without direct supervision.
  • Competent/confident in using and supporting the use of technology, including data entry.
