Job Title: Community Health Nurse

Derbyshire Healthcare NHS Foundation Trust is seeking a skilled Community Health Nurse to join our team. As a Community Health Nurse, you will be responsible for delivering health visiting services to a defined corporate caseload within Derby City.

Main Responsibilities:

  • Provide quality evidence-based public health nursing service to support and enable families and children to make informed choices about their health care and promote health and wellbeing.
  • Lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Universal plus, Universal Partnership plus models of service delivery.
  • Work collaboratively with other agencies and partners to ensure a seamless delivery of services.

Requirements:

  • Registered nurse or soon-to-qualify
  • Excellent communication skills
  • Experience of working as part of a team
