Job Summary

We are seeking an enthusiastic and skilled Community Nursing Sister to join our team at Midlands Partnership NHS Foundation Trust. As a Community Nursing Sister, you will play a vital role in providing high-quality care to patients in their own homes, working closely with GPs and multidisciplinary teams to address a range of care issues across various health conditions and preventing avoidable hospital admissions.

Main Duties

As a Community Nursing Sister, you will be responsible for delivering a high-quality service, working independently in a community setting, participating in the ongoing assessment, planning, implementation, and evaluation of programmes of care, adhering to policies and procedures. You will also be required to work closely with the Clinical Lead and Operational Lead to manage and coordinate aspects of care and service provision, including case management within the team.
