Job Summary

We are seeking an enthusiastic and skilled Community Nursing Sister to join our team at Midlands Partnership NHS Foundation Trust. As a Community Nursing Sister, you will play a vital role in providing high-quality care to patients in their own homes, working closely with GPs and multidisciplinary teams to address a range of care issues across various health conditions.

Main Responsibilities

As a Community Nursing Sister, your key responsibilities will include:

  • Delivering high-quality care to patients in their own homes, working independently and as part of a team
  • Participating in the ongoing assessment, planning, implementation, and evaluation of programmes of care, adhering to policies and procedures
  • Working closely with GPs and multidisciplinary teams to address a range of care issues across various health conditions
  • Supporting the Operational Lead and Clinical Lead in managing and coordinating aspects of care and service provision, including case management within the team

Requirements

To be successful in this role, you will need to be:

  • NMC registered
  • Able to demonstrate a commitment to delivering high-quality targeted personal care
  • Experienced in working in the community
  • Able to work independently and as part of a team
  • Able to communicate effectively with patients, families, and healthcare professionals
