Job Summary

We are seeking an enthusiastic and skilled Community Nursing Sister to join our team at Midlands Partnership NHS Foundation Trust. As a Community Nursing Sister, you will play a vital role in providing high-quality care to patients in their own homes, working closely with GPs and multidisciplinary teams to address a range of care issues across various health conditions.

Main Duties

The Community Intervention Service provides care to patients in their own homes, focusing on prevention and addressing the needs of frail older people with long-term conditions, as well as acutely unwell patients in the community. As a Community Nursing Sister, you will deliver a high-quality service, working independently in a community setting, participating in ongoing assessment, planning, implementation, and evaluation of care programs, adhering to policies and procedures.

Requirements
  • Nursing Degree
  • NMC registration
  • Post-qualification courses in long-term conditions or relevant subjects
