Nurse Case Manager for Hospice Admissions

Community Hospice & Palliative Care is seeking a skilled Nurse Case Manager to join our team in Jacksonville, FL.
About the RoleWe are looking for a dedicated and compassionate RN to facilitate speed to care by providing preadmission and admission services. As a Hospice Admission Register Nurse, you will collaborate with internal and community partners to resolve barriers to admission, provide education on hospice services and philosophy, and ensure the best outcome for patients, families, and referral sources.
Responsibilities- Promote Community Hospice as the preferred provider of hospice care in our service area.
- Sell the benefits of hospice care and resolve patient/family objections.
- Provide information about Community Hospice services and philosophy to patients, caregivers, physicians, and other community referral sources.
- Implement Medicare Local Coverage Determination to assess patients for hospice eligibility.
- Collect and communicate appropriate information to/from admission physician to determine hospice eligibility.
- Perform initial physical assessments, identify immediate needs, and apply nursing interventions.
- Facilitate admission to Community Hospice and provide emotional support to patients and families/caregivers.
- Maintain confidentiality and comply with HIPAA regulations.
- Current licensure as a Registered Nurse (RN) in the state of Florida.
- Ability to work independently and collaboratively as part of a team.
- Strong communication and interpersonal skills.
- Experience in hospice or palliative care preferred.
- A competitive salary of $60,000 - $80,000 per year, depending on experience.
