Job Title: RN Home Admissions

Job Summary:

Community Hospice & Palliative Care is seeking a skilled and compassionate Registered Nurse to join our team as a Home Admissions RN. As a key member of our admissions team, you will be responsible for promoting Community Hospice as the preferred provider of hospice care in our service area, educating patients and families about our services, and facilitating the admission process.

Responsibilities:

  • Promote Community Hospice as the preferred provider of hospice care in our service area.
  • Provide education and support to patients and families about our services and philosophy.
  • Facilitate the admission process, including assessing patient eligibility and coordinating with healthcare providers.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Provide emotional support and comfort to patients and families during times of need.
  • Document patient information and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current licensure as a Registered Nurse in the state of Florida.
  • Current CPR certification.
  • Minimum of two years' experience in a hospital or long-term care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.

Preferred Qualifications:

  • Certification in Hospice and Palliative Nursing.
  • Experience working in hospice or palliative care.
  • Knowledge of Medicare and Medicaid regulations.
