Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at Community Hospice & Palliative Care. As a RN Baylor, you will provide high-quality patient care and support to individuals and families in need of end-of-life care and palliative services.

Responsibilities:

  • Conduct thorough assessments of patients to identify their needs and develop effective care plans.
  • Provide skilled nursing care, including symptom management, wound care, and medication administration.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient information accurately and timely in the electronic medical record.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care planning.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements:

  • Current licensure as a Registered Nurse in the State of Florida.
  • Minimum of two years of experience in acute care, oncology, or home health care settings.
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment.
  • Ability to work independently and make sound judgments in a fast-paced environment.

Work Environment:

This is a home care position, requiring the use of a personal vehicle and reliable transportation. The work environment is dynamic and fast-paced, with a focus on providing high-quality patient care and support.
