Key Responsibilities:
Weekend Coverage:
Delivering High-Quality Patient Care:
- Conduct thorough assessments of patients to ensure effective symptom management.
- Utilize exceptional nursing skills, communication skills, and emotional intelligence to effectively address patient needs.
- Administer treatments and interventions in accordance with the plan of care and physician's orders.
- Provide patients and caregivers with valuable information and education regarding disease progression and end-of-life care.
- Offer instruction to patients and caregivers in accordance with the plan of care or changes in the plan of care.
- Collaborate with patients, families, caregivers, and practitioners to discuss goals of care and changes in treatment plans.
- Adhere to precautions outlined in the plan of care.
- Provide emotional support to patients and caregivers during all interactions.
- Conduct follow-up and collaboration calls as needed or requested by the team.
Minimum Qualifications:
- Possession of a degree from a diploma nursing program or a Bachelor's degree in Nursing.
- Minimum of two (2) years of experience in an acute care setting, oncology, or home health care setting.
- Skilled in teaching, communication, and listening skills, assessment skills, central/peripheral IV skills, and working within a multidisciplinary team framework.
Required Licenses/Certifications:
- Current license as a Registered Nurse in the State of Florida.
- Current CPR certification.
- Current State of Florida driver's license with a driving record acceptable to Community Hospice & Palliative Care's insurance carrier and reliable transportation.
Work Environment and Physical Requirements:
Home care position for 8 to 12 hour shifts. Use of own vehicle required. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
