Job Summary:

Community Hospice & Palliative Care is seeking a skilled Registered Nurse to join our team as a Clinical Nurse Specialist - Inpatient Care. This role will provide high-quality patient care and support to patients and their families in a hospital setting.

Key Responsibilities:

  • Conduct thorough assessments of patients to identify their physical, functional, and psychosocial needs.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Provide emotional support and education to patients and their families to promote independence and self-care.
  • Supervise and mentor hospice aides to ensure high-quality patient care.
  • Stay up-to-date with the latest clinical guidelines and best practices in hospice care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

  • Current licensure as a Registered Nurse in the state of Florida.
  • Current CPR certification.
  • One year of experience in medical-surgical nursing, preferably in a hospice setting.

Preferred Qualifications:

  • Bachelor's degree in Nursing.
  • One year of experience in hospice nursing.
