Palliative Care Specialist

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our Community Hospice team as a Palliative Care Specialist. In this role, you will provide high-quality, patient-centered care to individuals and families in their homes, focusing on symptom management, education, and support.

Key Responsibilities:

  • Conduct thorough assessments of patients' needs and develop individualized care plans.
  • Provide skilled nursing interventions, including wound care, medication management, and education on disease management.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and effective communication.
  • Document all interactions and interventions in the electronic medical record.
  • Participate in ongoing education and training to stay current on best practices and evidence-based care.

Requirements:

  • Current licensure as a Registered Nurse in the State of Florida.
  • Minimum of two years of experience in acute care, oncology, or home health care settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.

Work Environment:

This is a home care position, requiring the use of your own vehicle and a valid Florida driver's license. You will work 8-12 hour shifts, with flexibility to accommodate patient needs.
