Community Health Nurse

We are seeking an experienced Community Health Nurse to join our team in the NHS. As a key member of our community nursing service, you will provide high-quality, skilled nursing care to patients at home and contribute to rapid response, rehabilitative, and post-acute care in various community settings.
Main Responsibilities- To deliver evidence-based nursing care to patients in diverse community settings, utilizing enhanced skills and knowledge to assess physical and psycho-social needs.
- To develop comprehensive and concordant care plans with patients, carers, and the multidisciplinary team as needed, ensuring patient safety and well-being.
- To conduct risk assessments, establishing the safety or otherwise of patients' ability to stay at home or return home safely from another care setting.
Our ideal candidate will be an NMC registered, UK-based nurse with experience in undertaking comprehensive health needs assessments, writing care plans, and providing acute care responses to frail and elderly patients. They will also possess excellent communication and interpersonal skills, enabling them to effectively communicate complex information with patients, relatives, and healthcare professionals.
