Central London Community Healthcare NHS Trust is a leading provider of community healthcare services in London and Hertfordshire. We are currently seeking a highly skilled Rapid Response Nurse to join our team at Sudbury Primary Care in Wembley.

About the Role:
  • This is a challenging yet rewarding position that requires a high level of clinical expertise and a passion for delivering exceptional patient care.
  • You will work as part of a dynamic team to provide rapid response, rehabilitative and post-acute care to patients in their homes and community settings.
Key Responsibilities:
  1. To deliver high-quality nursing care to patients with complex health needs, utilising your specialist skills and knowledge to assess physical and psycho-social needs.
  2. To develop and implement comprehensive care plans in collaboration with patients, carers and the multidisciplinary team.
  3. To make risk assessments and provide necessary nursing and supportive packages to ensure safe care for patients at home or in other care settings.
  4. To work autonomously and as part of a team to provide evidence-based clinical interventions, including intravenous therapy, anticoagulation monitoring and palliative care.
Requirements:
  • A registered nurse with current NMC registration and recent post-registration experience in community or A&E nursing.
  • Qualifications in community and/or accident and emergency nursing, and evidence of ongoing professional development.
  • Extensive range of clinical skills, including IV therapy, venepuncture and cannulation.
  • Experience of working with people with complex health needs and implementing evidence-based care in practice.
