Job Description

We are seeking a highly skilled and motivated individual to fill the role of NHS CHC Learning Disability Clinical Assessor. This is an exciting opportunity to join our team and contribute to delivering high-quality care to our community.

About the Position
  • This is a full-time position based at Sudbury Primary Care Centre in North West London.
  • You will work closely with our multidisciplinary team to assess patients' needs and develop personalized care plans.
  • Your expertise in learning disability nursing or allied health care professional will be invaluable in providing high-quality care.
Key Responsibilities
  1. Evaluate patient needs and develop comprehensive care plans.
  2. Collaborate with healthcare professionals to ensure seamless care delivery.
  3. Advocate for patients' rights and preferences.
Requirements
  • Degree in Nursing (RNLD) or registered allied health care professional.
  • Experience working in complex, multi-disciplinary environments.
  • Strong knowledge of The National Framework for NHS Continuing Health Care.
  • Excellent communication and interpersonal skills.
