About the Role

We are seeking a highly skilled and compassionate Carer Support Worker to join our Community Mental Health Team in Stockport. As a key member of our team, you will play a vital role in providing integrated health and social care to carers of individuals with severe and enduring mental disorders.

Key Responsibilities

  • Support carers practically and emotionally, ensuring they are well-informed and supported with their individual health and well-being needs.
  • Work closely with clinical teams to offer support to carers in their own right.
  • Support carers to attend clinical team reviews, ward rounds, and CPA meetings.
  • Partner with Local Authority, the voluntary and community sector, and other NHS agencies to increase carer identification and recognition.
  • Ensure the role of unpaid carers is recognized, supported, and valued, promoting the Triangle of Care standards and principles.
  • Provide carers with information and advice about services within and external to the organization, contributing to their caring role and increasing knowledge regarding care and treatment processes.
  • Work collaboratively with carers and clinical teams to ensure carers feel involved and supported in the care and treatment of their loved one, in accordance with Service User consent, Trust Strategic Priorities, and Carers Engagement Procedures.
  • Have knowledge of the identification and support required for young carers.
  • Facilitate the delivery of carer support in a group setting where required.
