About the Role

We are seeking a highly motivated and compassionate Carer Support Worker to join our Community Mental Health Team in Stockport. As a key member of our team, you will play a vital role in supporting carers of individuals with severe and enduring mental disorders.

Key Responsibilities
  • Provide emotional and practical support to carers, ensuring they are well-informed and supported in their caring role.
  • Work closely with clinical teams to offer support to carers in their own right.
  • Support carers to attend clinical team reviews, ward rounds, and CPA meetings.
  • Partner with Local Authority, the voluntary sector, and other NHS agencies to increase carer identification and recognition.
  • Promote the Triangle of Care standards and principles, and other carer-specific initiatives to services and carers.
  • Provide carers with information and advice about services within and external to the organisation, contributing to their caring role and increasing their knowledge of care and treatment processes.
  • Work collaboratively with carers and clinical teams to ensure carers feel involved and supported in the care and treatment of their loved one, in accordance with Service User consent, Trust Strategic Priorities, and Carers Engagement Procedures.
  • Have knowledge of the identification and support required for young carers.
  • Facilitate the delivery of carer support in a group setting where required.
