We are seeking a highly motivated and compassionate Carer Support Worker to join our Community Mental Health Team in Stockport. As a key member of our team, you will play a vital role in supporting carers of individuals with severe and enduring mental disorders.
Key Responsibilities- Provide practical and emotional support to carers, ensuring they are well-informed and supported in their caring role.
- Work closely with clinical teams to offer support to carers in their own right, promoting their involvement and engagement in the care and treatment of their loved ones.
- Support carers to attend clinical team reviews, ward rounds, and CPA meetings, ensuring their voices are heard and valued.
- Collaborate with local authorities, the voluntary sector, and other NHS agencies to increase carer identification and recognition.
- Facilitate the delivery of carer support in a group setting, promoting the Triangle of Care standards and principles.
- Provide carers with information and advice about services, both within and external to the organization, to enhance their caring role and knowledge of care and treatment processes.
