As a Support Practitioner, you'll work closely with colleagues to create and implement tailored support plans that meet the unique needs of each service user.

Main duties and responsibilities:

  • Engaging with service users to understand their needs and preferences
  • Developing and implementing individualized support plans
  • Coordinating daily activities and providing emotional support

We anticipate the salary for this role to be around £23,000-£27,000 per annum, depending on qualifications and experience.
