About the Role
The PCN Health Coach will play a critical role in engaging patients and using health coaching techniques to support them to take an active role in their health and wellbeing.
The post holder will have a key role in helping to raise the local population's awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals.
The role will be an integral part of the general practice team and the individual will be working across practices within the PCN to enhance individual patient case management, follow up and ongoing support.
Main Responsibilities
- To work as part of a multi-disciplinary team in a patient-facing role to assess and treat patients using expert knowledge.
- To be responsible for the assessment of new patients with subsequent production and completion of individual care plans.
- To coach and motivate patients through multiple sessions to identify their needs, set goals and support them to implement their personalised health and care plan.
- To provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in managing their own health and wellbeing.
Requirements
- Evidence of a CPD portfolio.
- Experience of working with groups.
- Experience in health and nutrition work with patients.
- Demonstrable experience of working with a wide range of ages and patient demographics.
