About the Role
The PCN Health Coach will work as part of a multi-disciplinary team to support patients with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing.
The role will involve increasing patient ability to access and utilise community support, providing access to self-management education, peer support, and social prescribing.
The PCN Health Coach will take an approach that considers the whole person in addressing existing issues and encourages proactive prevention of new illnesses.
The role will be to appreciate patients' circumstances in a non-judgemental manner, resulting in the need for positive communication and negotiation skills that will support the patients' personal choices.
Main Responsibilities
- To work as part of a multi-disciplinary team in a patient-facing role to assess and treat patients using expert knowledge.
- To be responsible for the assessment of new patients with subsequent production and completion of individual care plans.
- To coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.
- To provide personalised support to individuals, their families, and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in managing their own health and wellbeing.
Person Specification
Essential
- Evidence of a CPD portfolio.
- Experience of working with groups.
- Experience in health and nutrition work with patients.
- Demonstrable experience of working with a wide range of ages and patient demographics.
Desirable
- Broad knowledge of general practice.
- An appreciation of the new NHS landscape, including relationships between individual practices, PCNs, and the commissioners.
- Experience with alcohol or drugs intervention.
- Smoking cessation training (or working towards completion).
