Care Manager Position

Active Care Group's Care in the Home division continues to thrive, providing safe, reliable, and outcome-based care to clients through our exceptional support workers, clinicians, and management team.
We are seeking an experienced Care Manager with NVQ 4 or above in Health and Social Care to join our friendly and supportive team. This is an exciting opportunity to contribute to our Care in the Home team as we continue to grow the division.
As Care Manager, you will have ownership of care packages within the Shropshire, Cheshire, and surrounding areas. This role is extremely rewarding as the care we provide allows clients to remain in their own homes and maintain as much independence as their condition allows. Regular travel to visit clients in your area is required, and we need a driver with their own car.
Active Care Group has over 25 years of specialist experience in providing high-quality complex care for people in their own homes, taking into account not just their clinical needs but the whole person, including their personal, social, and emotional needs. Our packages of care are tailored to fit each person's care needs, preferences, and lifestyle. We also pride ourselves on the fact that our care extends beyond the individual to their wider family and friends, as we understand the wider impact complex conditions can have.
Key Responsibilities:
- Hours per week, predominantly Monday-Friday, with flexibility to work varied hours and days
- Participation in an on-call rota on nights/weekends
Key Responsibilities (continued):
- Ownership of care packages in your area
- Collaboration with the Business Development Manager to review suitable referrals
- Production of suitable assessment reports for care packages for clients
- Building excellent working relationships with clients, families, and external bodies
- Ensuring high-quality client-focused care at all times
- Support with recruitment and training for Support Workers for care packages in your area
- Accuracy and timeliness of all records
Requirements:
- NVQ 4 or above in Health and Social Care
- Driver with access to own car
- Passion for delivering high-quality client-focused care
- Experience in care at Team Leader level or above
- Previous management experience would be an advantage
- Excellent verbal and written communication skills
- Positive attitude and commitment to continuous improvement and quality
