Job Title: Adult Placement Officer

We are seeking a skilled and organized Adult Placement Officer to join our team at Essential Employment. As a Placement Officer, you will play a crucial role in arranging packages of care in the community for individuals leaving hospital.

Key Responsibilities:

  • Work closely with health colleagues to coordinate care packages
  • Develop and maintain relationships with healthcare providers and community organizations
  • Ensure seamless transitions for individuals leaving hospital
  • Collaborate with the team to achieve care goals and objectives

Requirements:

  • Ability to work at pace and prioritize tasks effectively
  • Strong communication and interpersonal skills
  • Organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information

Working Arrangements:

  • Part-time position with occasional weekend cover
  • Average 18 hours per week or 36 hours across two weeks
  • Usual rota: Monday and Tuesday in Week one, Tuesday, Wednesday, and Thursday in Week two
