Job Title: Adult Placement Officer

We are seeking a skilled Adult Placement Officer to join our team at Essential Employment. As a Placement Officer, you will play a crucial role in arranging packages of care in the community for individuals leaving hospital.

Key Responsibilities:

  • Work closely with health colleagues to coordinate care packages
  • Develop and maintain relationships with healthcare providers and community organizations
  • Ensure seamless transitions for patients from hospital to community care
  • Collaborate with the care team to identify and address individual needs

Requirements:

  • Ability to work at pace and prioritize tasks effectively
  • Strong communication and interpersonal skills
  • Organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information

Working Arrangements:

This is a temporary role with a part-time schedule, requiring occasional weekend cover. The successful candidate will work an average of 18 hours per week, with a regular rota of Monday and Tuesday one week, and Tuesday, Wednesday, and Thursday the next.
