About the Role

This is an exciting opportunity to join our multidisciplinary team as a Social Worker in Devon. As a key member of our Community Health & Social Care Team, you will be responsible for assessing individuals' care needs and identifying suitable outcomes.

Key Responsibilities
  • To work independently and as part of a team to provide high-quality social work services to adults over 18 with physical disabilities.
  • To conduct assessments and develop plans that support individuals to achieve their goals and improve their well-being.
  • To build strong relationships with individuals, families, and carers to understand their needs and preferences.
Requirements

We are looking for a qualified and experienced Social Worker who possesses:

  • Robust knowledge of the Care Act 2014 and other relevant legislation.
  • A confident assessment approach and ability to work independently.
  • Excellent communication and interpersonal skills.
  • An understanding of the importance of promoting diversity, equality, and inclusion.
