We are seeking a skilled Social Care Specialist to join our team at Ackerman Pierce Ltd. As a key member of our Continuing Healthcare Team, you will play a vital role in providing high-quality care coordination services to individuals who meet the eligibility criteria for Continuing Health Care (CHC).

**Key Responsibilities:**

* Provide a comprehensive care coordination approach to the management and support of individuals who meet the eligibility criteria for CHC
* Conduct thorough reviews of care packages to ensure they meet the individual's needs
* Undertake comprehensive social care assessments and reports, including detailed Support Plans as appropriate
* Collaborate with families, carers, and communities to empower them to make informed decisions and contribute to service planning
* Assess and reassess complex health and social care needs
* Facilitate the completion of Decision Support Tools (DST) to establish eligibility for healthcare
* Complete Continuing Healthcare Checklists
* Participate in Multi-Disciplinary Team (MDT) meetings to discuss individual care plans
