Job DescriptionWe are seeking a qualified social worker to join our Adults Learning Disabilities and Care Management Team in Enfield. The successful candidate will work as part of an integrated health and social care service, providing support to people with learning disabilities and their carers.
- Develop and implement support plans that enhance service user development and demonstrate best value and community inclusion.
- Conduct reviews of existing learning disabilities service users' current packages of care.
- Work collaboratively with colleagues and external agencies to provide a comprehensive service.
Work Details:
- 36 hours per week (flexible by negotiation).
- 2 Month Temporary Contract.
- Start date subject to DBS and reference clearance.
