Job Overview

We are seeking an experienced and enthusiastic Senior Mental Health Practitioner to join our Hindley Primary Care Network Mental Health Team. This role is open to Registered Mental Health Nurses (RMN), Occupational Therapists (OT), and Social Workers.

Main Responsibilities

The Senior Mental Health Practitioner will work closely with the Neighbourhood Teams, Social Care teams, VCSE organisations, and the Living Well service to strengthen the care and support being offered. They will have the opportunity to interface with the Living Well MDT to provide a keyworker role to individuals with complex needs. The Senior Mental Health Practitioner will be a conduit between Primary Care, Living Well, secondary care, and other agencies, ensuring warm introductions and smooth seamless journeys for service users.

Key Duties
  • Offer weekly slots for appointments and carry a small caseload from each practice within the PCN, assessing the mental health and social care needs of individuals.
  • Provide clinical and management supervision for a small number of staff.
  • Work with individuals who are not meeting the threshold for community mental health teams and who are not responding to primary care mental health interventions or are not appropriate for IAPT.

Person Specification

Essential Criteria

  • Qualified and registered RMN, OT, or Social Worker.
  • Postgraduate training in a relevant subject.
  • Relevant post-qualification experience in mental health services.
  • Post-qualification experience of supervising staff.

Desirable Criteria

  • NMP or willingness to train.
  • Experience of working in primary care.
