About the Role

We are seeking a skilled and compassionate Mental Health Practitioner to join our team at the Living Well Service in Wigan. As a key member of our multidisciplinary team, you will play a vital role in providing timely access to mental health services for individuals in need.

The successful candidate will have experience of working in mental health community services, with a strong understanding of trauma-informed care and the importance of applying this in practice. You will be responsible for assessing mental health needs, planning onward care, and working closely with VCSE services to provide comprehensive support.

Key Responsibilities
  • Provide high-quality mental health care to individuals in the community, working flexibly to meet their needs.
  • Assess mental health needs, plan onward care, and develop robust risk management and pathway planning.
  • Work closely with VCSE services, other agencies, and professionals to provide comprehensive support.
  • Travel across Wigan to other Primary care network areas, visiting individuals in their homes or community settings.

Person Specification
  • Registered Mental Health Nurse with post-registration experience in mental health community services.
  • Non-Medical prescribing qualification.
  • Strong understanding of trauma-informed care and its application in practice.
