About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health assessments and interventions to patients referred from the Emergency Department and Medical wards.

Key Responsibilities
  • Conduct comprehensive mental health assessments and risk assessments to identify patients' needs and develop effective risk management plans.
  • Provide brief interventions and signpost patients to appropriate mental health services, ensuring seamless transitions and continuity of care.
  • Work collaboratively with the multidisciplinary team to ensure effective interfacing between teams, other mental health services, and agencies such as the Police, NWAS, and local authority.
  • Support the development of the team's gateway and assessment role, contributing to the expansion of our service offer.
  • Provide information and clinical support to police and ambulance staff to help them make informed decisions.
Person Specification

We are looking for a candidate with:

  • Professional registration in health or social care with up to-date accreditation/CPD.
  • Substantial post-registration experience working in acute mental health services.
  • Experience in working with children/young people.
  • Knowledge of national legislation and guidance, including the Mental Health Act and Mental Capacity Act.
  • Ability to identify and coordinate effective risk assessment and management.
