Position Overview:
We are seeking a dedicated and skilled Community-Based Rehabilitation Specialist to become a vital part of our well-organized and proactive Reablement Intervention team.

Our organization provides a full-time employment opportunity with the flexibility to work remotely, a balanced caseload, and opportunities for professional development.

This role is initially for a six-month period, with the possibility of extension.

Key Responsibilities:
  • Establishing personalized goals for clients.
  • Planning for client discharge and providing necessary equipment.
  • Assessing functional independence and identifying manual handling and environmental access requirements.
  • Managing risks to ensure safety in accordance with local authority regulations and guidelines.
  • Collaborating with both internal and external partners to achieve optimal outcomes for service users.
  • Offering expert advice and conducting thorough assessments.
  • Recommending necessary adaptations and modifications.
  • Encouraging client independence by prescribing minor aids, equipment, and creating moving and handling plans.
  • Engaging with other services and making appropriate referrals.

It is essential to have a strong awareness of safeguarding vulnerable individuals and to uphold the principles of dignity and respect in all interactions.

Qualifications:
To be successful in this role, candidates should possess:

  • Previous experience as a Rehabilitation Specialist in a community setting.
  • A minimum of two years of experience in Adult Social Care.
  • Experience with equipment provision to facilitate home working.
  • Manual handling expertise.
  • HCPC Registration.
  • Enhanced DBS/CRB clearance (can be arranged).
  • Eligibility to work in the UK.
  • Relevant qualifications in Occupational Therapy.
