About the Role

We are seeking a highly skilled and motivated Band 5 Physiotherapist to join our Community Stroke Service team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality physiotherapy services to patients in their homes and in our stroke rehabilitation units.

Key Responsibilities
  • Provide comprehensive physiotherapy assessments, diagnoses, and treatments to patients with diverse presentations and complex physical and psychological conditions.
  • Develop and implement individualized rehabilitation plans in collaboration with patients, carers, and other healthcare professionals.
  • Participate in multidisciplinary team meetings and case conferences to ensure seamless care coordination and patient-centered goals.
  • Contribute to the development and implementation of evidence-based practice guidelines and protocols.
  • Provide education and training to nursing staff on basic mobility techniques and other relevant topics.
