Key Responsibilities- Provide comprehensive assessments and develop treatment plans for patients with stroke and other medical conditions.
- Liaise with a range of healthcare professionals, including consultants, nurses, and therapists, to ensure seamless care pathways.
- Contribute to the development of stroke and community practice, aligning with best practice and nationally accredited standards.
- Collaborate with other services and agencies, including social care, safeguarding, and the voluntary sector, to ensure comprehensive patient care.
- MB BS or equivalent medical qualification.
- Experience of working in older persons care.
- Compassionate, listening, professional, and patient-focused approach.
- Good organisational skills and ability to manage challenging situations.
