About the Role
We are seeking a highly skilled Clinical Pharmacist to join our clinical adult mental health team at the Whiteleaf Centre in Aylesbury. As a specialist in mental health pharmacy, you will be responsible for developing excellent clinical relationships with patients, carers, and healthcare professionals, and supporting them with any medication issues they may have.
Main Responsibilities
- Directly support patients, prescribers, and other healthcare professionals via attendance at multidisciplinary team meetings, carrying out medication reviews, and ensuring the correct application of national and local guidance for therapies involving medicines.
- Be responsible for the development of excellent clinical relationships with patients, carers, and healthcare professionals, and supporting them with any medication issues they may have.
