Clinical Specialist Pharmacist

An exciting opportunity has arisen for a rotational Band 7 pharmacist to join our dynamic clinical pharmacy team in Buckinghamshire Healthcare NHS Trust.
The post holder will rotate through various clinical specialties, including emergency and general medicine, surgery and critical care, women and children's services, and our National Spinal Injuries Centre. This is a unique opportunity to work closely with multidisciplinary teams to deliver high-quality pharmaceutical care, advice, and medicines management services.
Main Duties- To deliver a specialist pharmacy service to different care groups in the trust.
- Identification and prioritization of high-risk patients requiring specialist pharmacist input and delivering proactive and prompt pharmaceutical advice.
- Undertaking medicines management duties, medicines reconciliation, optimization, improving prescribing practices, and supporting timely discharge through the validation and supply of medication.
- Delivery of specialist and accurate information and educational training to medical and nursing staff and patients regarding medications, therapeutic effects, monitoring, and potential adverse effects.
- Participation in the provision of a sessional dispensary service for inpatients and outpatients at any pharmacy department within the trust.
- Contribution to guideline development, medicines safety, drug budget management, and formulary applications.
