About the Role

We are seeking a highly motivated and experienced Assistant Practitioner to join our Mental Health Liaison Team at Southern Health NHS Foundation Trust. As an Assistant Practitioner, you will play a vital role in supporting individuals with mental health needs and learning disabilities in our acute hospital settings.

Key Responsibilities
  • Undertake keyworker roles for individuals with mental health needs and their carers, providing support and guidance in emergency departments and acute wards.
  • Support the triage and diversion of individuals presenting with mental health crises, ensuring their physical health needs are met.
  • Collaborate with multidisciplinary teams to provide mental health assessments and support in emergency departments and acute wards.
  • Develop and review care plans, identifying and organizing resources as necessary.
  • Work closely with service users, families, and carers to provide a recovery-focused approach.

Requirements
  • NVQ Level 3 in Health and Social Care or equivalent.
  • Experience of working within a team environment and using IT packages.
  • Previous experience of working in the NHS or healthcare sector.
