About the Role

We are seeking a skilled Social Worker to join our Ageing Well West Service team in Southend-on-Sea. As a key member of our team, you will be responsible for providing assessment and review functions for older adults and those with mental health needs.

Key Responsibilities

  • Conduct comprehensive assessments and reviews to identify the needs of older adults and develop person-centred care plans
  • Work collaboratively with health colleagues to prevent unnecessary admissions to long-term care and support interdependence within the community
  • Support adults and their carers as a 'change agent' to build on their strengths, assets, and abilities to self-manage
  • Develop and maintain effective relationships with service users, carers, and other stakeholders

Requirements

  • Recognised Social Work qualification and registration with Social Work England
  • Current experience of working in a social care setting, preferably with older adults
  • Ability to work in the UK and have a valid visa (if applicable)
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
