About the Role

We are seeking a dedicated and qualified Social Worker to join our Adults Community Team (West) in Southend-on-Sea. As a key member of our team, you will play a vital role in enhancing the well-being of individuals in our community.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' care and support needs.
  • Develop and implement personalized care plans in collaboration with service users and their families.
  • Provide practical support and guidance to individuals in the community, promoting independence and enhancing overall quality of life.
  • Work closely with healthcare professionals and external agencies to coordinate holistic care.
Requirements
  • Qualified Social Worker registered with Social Work England.
  • Experience in adult social care, with a focus on community-based services.
  • Strong knowledge of relevant legislation and policies in adult social work.
