Crisis Resolution Home Treatment Nurse Specialist

We are seeking a highly skilled and experienced Mental Health Nurse to join our Crisis Resolution Home Treatment Team as a Crisis Resolution Home Treatment Nurse Specialist. The successful candidate will be responsible for providing high-quality, patient-centered care to individuals experiencing mental health crises in their own homes.
Key Responsibilities- Provide urgent and emergency triage, advice, and assessment for people in a mental health crisis.
- Undertake home treatment visits as indicated by the multidisciplinary team (MDT).
- Provide and deliver highly skilled nursing care, including full mental health assessment, care planning, and care delivery.
- Recommend and refer, where treatment is indicated, to the most appropriate team/service.
- Participate in Mental Health Act assessments to consider alternatives to formal hospital admission.
