Crisis Resolution Home Treatment Nurse Specialist

We are seeking a highly skilled and experienced Mental Health Nurse to join our Crisis Resolution Home Treatment Team as a Band 6 Nurse. The successful candidate will be responsible for providing urgent and emergency triage, advice, and assessment for individuals in a mental health crisis, as well as undertaking home treatment visits as indicated by the Multi-Disciplinary Team (MDT).
Key Responsibilities- Provide a clinical service that is user-focused, accessible, and responsive 24/7.
- Triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
- Deliver highly skilled nursing care, including full mental health assessment, care planning, and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
- Recommend and refer, where treatment is indicated, to the most appropriate team/service.
- Undertake gatekeeping assessments for inpatient admissions and crisis house admissions.
- Work collaboratively with the crisis house to lead on care for admissions as a service.
- Facilitate early discharge from inpatient wards where clinically indicated.
- Deliver home-based treatment interventions for all clients, ensuring best practice is implemented.
- Participate in Mental Health Act assessments to consider alternatives to formal hospital admission.
